Method:The rat model of postmenopausal osteoporosis was established by bilateral ovariectomy, 60 female SD rats were randomly divided into sham operation group, model group, positive group (estradiol valerate tablet 0.05 mg·kg-1·d-1) and low, middle and high dose groups of Zuoguiwan (5.5,11,22 g·kg-1·d-1).After successful establishment of the model in the 13th week, intragastric administration (ig) was given once a day for a total of 12 weeks. After administration, the histomorphological changes of femur in rats were observed by hematoxylin-eosin (HE) staining, the bone mineral density (BMD) and bone mineral content(BMC) of femur were measured by dual energy X-ray apparatus, and the biomechanical properties of bone were measured by MTS Acumen3 biomechanical testing system. The contents of bone alkaline phosphatase (BALP), bone glaprotein(BGP),estradiol (E2) ,and tartrate-resistant acid phosphatase (TRAP), type Ⅰ procollagen N-terminal propeptide (PINP) in serum were detected by enzyme-linked immunosorbent assay (ELISA). Western blot was used to detect the protein level of Wnt2,β-catenin,low density lipoprotein related receptor protein 5 (LRP5) and the phosphorylation level of glycogen synthase kinase-3β(GSK-3β) in rat tibia. Result:Compared with sham operation group, the maximum load and stiffness of BMD,BMC, in the model group decreased significantly(P<0.01), the contents of E2 and PINP in serum decreased significantly(P<0.01), the content of BALP,BGP,TRAP increased significantly(P<0.01), the expression levels of Wnt2,p-GSK-3β Ser9,LRP5 and β-catenin protein in bone tissue decreased significantly(P<0.01), the trabecula of femur became thinner and thinner, the number of bone trabeculae decreased. Compared with model group, the maximum load and stiffness of BMD,BMC, in estradiol group and Zuoguiwan group were significantly increased (P<0.05,P<0.01), the contents of serum E2 and PINP were significantly increased (P<0.05,P<0.01), the content of BALP,BGP,TRAP was significantly decreased (P<0.01), and the expression level of Wnt2,p-GSK-3β Ser9,LRP5, β-catenin protein in bone tissue was significantly increased (P<0.05,P<0.01) , the trabeculae of femur became thicker, the number increased, the structure was basically clear. Conclusion:Zuoguiwan has a certain preventive and therapeutic effect on osteoporosis in ovariectomized rats, and its mechanism may be related to increasing the level of estrogen, activating Wnt/β-catenin signaling pathway, up-regulating the expression of Wnt2 and LRP5 protein, inhibiting the activity of GSK-3β, reducing the degradation of β-catenin, coordinating the dynamic coupling balance between bone formation and bone resorption, correcting the disorder of bone metabolism and improving bone morphology.

ABSTRACT

This study evaluated the outcome indicators of the randomized controlled trials(RCTs) of traditional Chinese medicine(TCM) in the treatment of postmenopausal osteoporosis(PMOP) to provide a reference for the related research. Four Chinese databa-ses(CNKI, VIP, Wanfang and CBM) and three English databases(Cochrane Library, EMbase and PubMed) were searched syste-matically to screen RCTs of TCM in the treatment of PMOP according to the pre-set criteria, and the quality of the included trials was evaluated by the Cochrane risk-of-bias tool. A total of 29 627 articles were initially retrieved, and finally, 43 articles were included, including 34 in Chinese and 9 in English. As revealed by the results, the outcome indicators of 43 RCTs were mainly divided into six categories, with biochemical indicators accounting for 54.59%, bone mineral density(BMD) for 26.57%, quality of life for 6.28%, fracture for 1.94%, safety for 5.31%(including adverse reactions or events) and others for 5.31%. Biochemical indicators showed the maximum occurrence, followed by BMD. Many problems were found in the selection of outcome indicators of the TCM RCTs in the treatment of PMOP, such as the confusion of primary and secondary outcome indicators, the lack of endpoint criteria and vital clinical outcome indicators substituted by intermediate indicators, inconsistent evaluation standard of syndrome curative efficacy and neglected blinding in the measurement of subjective outcome indicators. The problems also included importance given to the efficacy indicators instead of the adverse outcome indicators, unnormalized indicator name, large quantitative range of the indicators, unconventional application of TCM efficacy criteria, seldom used confidence interval, relative effect indicator and absolute effect indicator.

ABSTRACT

Objective:As the problem of global aging intensifies, postmenopausal osteoporosis (PMOP) has become a global health problem among females. At present, the commonly used biological agents have been proved not suitable for long-term use due to multiple adverse reactions. Several Meta-analyses have confirmed the good safety and effectiveness of kidney-tonifying method against PMOP,but its therapeutic mechanism remains unclear. The purpose of this Meta-analysis was to evaluate the effect of kidney-tonifying method on osteoclastogenesis inhibitory factor(OPG)/receptor activator of nuclear transcription factor (NF)-κB (RANK)/receptor activator of NF-κB ligand (RANKL) signaling pathway in PMOP animal model,so as to provide an experimental basis for the treatment of PMOP with kidney-tonifying method. Method:The related articles were retrieved from PubMed,Ovid Medline,Embase,China National Knowledge Infrastructure (CNKI),Chongqing Weipu Database for Chinese Technical Periodicals (VIP),and Wanfang Data Knowledge Service Platform with the retrieval time set from their inception to January 2020. The quality of each included article was evaluated using the SYRCLE's risk of bias tool. Then RevMan 5.3 was utilized for Meta-analysis according to the Cochrane systematic review methodology. Result:Thirty-two studies involving 619 rats were included. The quality score of these studies ranged from 3 to 5 points. The results of the Meta-analysis indicated obvious advantages of kidney-tonifying method in increasing bone mineral density (BMD)[standardized mean difference (SMD)=2.01,95% confidence interval(CI)=1.50-2.52,P<0.000 01]),serum OPG level (SMD=3.33,95% CI=2.59-4.07,P<0.000 01),and OPG mRNA expression (SMD=11.81,95% CI=7.49-16.13,P<0.000 01),promoting OPG protein production (SMD=4.95,95% CI=3.09-6.81,P<0.000 01),reducing serum RANKL(SMD=-4.88,95% CI=-6.01--3.75,P<0.000 01) and RANK levels (SMD=-7.30,95% CI=-9.53--5.07,P<0.000 01),and down-regulating RANKL (SMD=-6.22,95%CI=-8.95--3.49,P<0.000 01) and RANK mRNA (SMD=-3.18,95% CI=-6.19--0.18,P<0.05) expression and RANKL protein expression in bone tissue (SMD=-3.99,95% CI=-5.47--2.50,P<0.000 01). Conclusion:The kidney-tonifying method has been proved to possess potential advantages in regulating the balance of OPG/RANK/RANKL signaling pathway in PMOP animal model. Nevertheless, more large-sample sized, properly designed, and high-quality animal experiments are still needed for further verification.

ABSTRACT

Objective: To observe the mRNA levels of Orexin and its receptors in the hypothalamus of ovariectomized osteoporosis rats, in order to explore the pathogenesis of postmenopausal osteoporosis(PMOP) and the mechanism Zuoguiwan. Method: An osteoporosis model induced by ovariectomy was established in rats. Totally 32 female Sprague-Dawley (SD) rats were randomly divided into sham-operated group, ovariectomized model group, 17β-estradiol treated positive group, and Zuoguiwan group, with 8 rates in each group. After 12 weeks of intragastric administration, the bone mineral density (BMD) and trabecular microstructural changes of femur were detected by micro-CT (μ-CT), and the morphological changes of bone tissue were observed by hematoxylin-eosin staining (HE) staining. The markers of bone turnover in serum osteocalcin (OCN), N-terminal propeptide of type Ⅰ procollagen (PINP), tartrate-resistant acid phosphatase (TRAP) were measured using enzyme-linked immunosorbent assay (ELISA). The mRNA expressions of orexin, orexin receptor 1 (OX1R) and orexin receptor 2 (OX2R) were measured by Real-time PCR. Result: Compared with sham-operated group, the μ-CT showed that BMD, bone volume fraction(BV/TV), trabecular thickness(Tb. Th)and trabecular number(Tb. N)in ovariectomized model group were significantly decreased (PPN-terminal propeptide of type Ⅰ procollagen (PINP) levels decreased, whereas tartrate-resistant acid phosphatase (TRAP) content increased (PPPPPPPPPPConclusion: Decreased mRNA levels of Orexin and its receptors in the hypothalamus may be one of the mechanisms of PMOP. Zuoguiwan may correct the imbalance of bone metabolism, improve the trabecular microstructure and improve bone by up-regulating the mRNA expressions of Orexin and its receptors in the hypothalamus, density, so as to show a therapeutic effect on PMOP.
